Towards personalized nutrition in the control of refractory chronic headache

Abstract

Introduction: Refractory Chronic headache significantly affects the patient's quality of life, and only a minority is able to progress in the treatment to the use of botulinum toxin or Anti-CGRP (Calcitonin gene-related peptide) monoclonal antibodies. Our objective was to assess the effectiveness and the adherence of our diet in the control of this type of headache.

Materials and Methods: A no-controlled experimental study. A total of 24 patients with chronic headache (? 15 days a month) and refractory (failure to ?3 preventive drugs), >18 years old, followed a diet based on headache triggers removal, gluten-free and low glycemic index, with progressive reintroduction of food according to tolerance. The primary objective was the reduction of more than 50% in the monthly frequency of headaches, and the secondary objective was the adherence (measured by the number of dietary transgressions and the total time on diet).

Results: The primary objective was achieved in 83% of patients (p<0,001), of which 55% experienced a reduction in headache frequency between 50-80%, 35% between 81-99%, and 10% of patients were free from headaches. Adherence was rated as very good in 17%, good in 58%, and fair in 25%; with an average duration of 2 months for patients in which it was unsuccessful and more than 3 months for those whom it was effective.

Discussion: The structure of the diet, in addition to achieving appreciable effectiveness within a reasonable time frame, allowed for flexibility in terms of the patient's tastes and tolerance.

Conclusions: This dietary strategy could be a promising option for the treatment of chronic refractory headaches.
