Electroencephalogram and continuous EEG monitoring in status epilepticus

Abstract

The electroencephalogram especially with video (video-EEG) frequently underused is very useful for diagnosis and follow up of patients with status epilepticus, or when there is suspicion of status epilepticus.
Some EEG patterns that are key for diagnosis are of difficult interpretation, and require a neurologist or pediatric neurologist with training in clinical neurophysiology that can recognize them. The clinical neurophysiology training takes 1 to 2 years fellowship after the residency program.
The continuous video-EEG helps to define if a given patient is in status epilepticus , if the patient ‘s treatment is effective, if more medications are required, and finaly, if the status epilepticus has been resolved.
Some patients with status epilepticus progress to nonconvulsive status epilepticus, whith is difficult to recognize.
The clinical semiology goes from confusion to coma. In these patients a continuous video-EEG allows to see if the patient is still in status.
After the status is under control, an additional 24 to 48 hours are needed to be sure that there is no determine status recurrence, or if the patient needs a more aggressive treatment.
