Introducción: la migraña es una entidad de alto impacto en la población general, su prevalencia es de tres a cuatro veces más frecuente en mujeres y lleva consigo discapacidad, además de ser una condición que genera costos directos e indirectos, llega a impactar la funcionalidad de los pacientes.
Objetivo: presentar recomendaciones basadas en la evidencia para el tratamiento preventivo de la migraña episódica en población adulta.
Metodología: revisión sistemática de la literatura, dirigida al tratamiento preventivo de pacientes adultos con migraña episódica, desarrollada de acuerdo con la declaración PRISMA 2020, con evaluación de la certeza de la evidencia mediante el enfoque GRADE y asignación de clases y niveles de recomendación según el sistema AHA/ACC.
Resultados: candesartán, topiramato, propranolol, metoprolol, amitriptilina, divalproato de sodio/ácido valproico y flunarizina, se incluyeron en el grupo de opciones de primera línea. Las opciones en segunda línea fueron galcanezumab, rimegepant y atogepant.
Conclusión: se presentan las recomendaciones terapéuticas para el tratamiento de la migraña episódica y se integra evidencia científica según sistemas GRADE y AHA/ACC, con el fin de facilitar su aplicación clínica.
