Resumen

Introducción: la migraña es una entidad de alto impacto en la población general, su prevalencia es de tres a cuatro veces más frecuente en mujeres y lleva consigo discapacidad, además de ser una condi­ción que genera costos directos e indirectos, llega a impactar la funcionalidad de los pacientes.

Objetivo: presentar recomendaciones basadas en la evidencia para el tratamiento preventivo de la mi­graña episódica en población adulta.

Metodología: revisión sistemática de la literatura, dirigida al tratamiento preventivo de pacientes adul­tos con migraña episódica, desarrollada de acuerdo con la declaración PRISMA 2020, con evaluación de la certeza de la evidencia mediante el enfoque GRADE y asignación de clases y niveles de recomen­dación según el sistema AHA/ACC.

Resultados: candesartán, topiramato, propranolol, metoprolol, amitriptilina, divalproato de sodio/ácido valproico y flunarizina, se incluyeron en el grupo de opciones de primera línea. Las opciones en segun­da línea fueron galcanezumab, rimegepant y atogepant.

Conclusión: se presentan las recomendaciones terapéuticas para el tratamiento de la migraña epi­sódica y se integra evidencia científica según sistemas GRADE y AHA/ACC, con el fin de facilitar su aplicación clínica.
