El rol de la terapia física en los trastornos motores funcionales: hacia un abordaje estandarizado y multidisciplinario

Resumen

Introducción: los trastornos motores funcionales (TMotF) son una causa frecuente de discapacidad y suelen afectar a población joven y en edad laboral, por lo que se asocian a costes elevados sociosani­tarios. La rehabilitación física tiene un rol importante en su abordaje, sobre todo en aquellos con sín­tomas motores predominantes, lo que favorece el reentrenamiento motor y restaura la funcionalidad, además, ha demostrado resultados efectivos y duraderos.

Materiales y métodos: artículo de revisión narrativa en el que se describe la evidencia sobre la fisiote­rapia especializada en trastorno neurológico funcional.

Resultados: en los últimos años, la evidencia sobre fisioterapia en TMotF se consolidó con estudios más robustos y enfoques interdisciplinarios. Las revisiones sistemáticas revisadas (2023-2024) con­firmaron que la fisioterapia mejora la movilidad, el equilibrio y la calidad de vida en TMotF, aunque la heterogeneidad de los protocolos limita la generalización.

Discusión: los enfoques multidisciplinarios que involucran la rehabilitación especializada temprana y la selección adecuada del paciente se asocian con mejores resultados. El tratamiento fisioterapéutico en los TMotF tiene como objetivo general favorecer la neuroplasticidad adaptativa y reforzar los pa­trones normales de movimiento mediante el reentrenamiento motor, orientado a la automatización. El tratamiento fisioterapéutico tiene tres componentes principales: la educación, el reentrenamiento motor y el automanejo.

Conclusiones: a pesar de los avances, persisten retos metodológicos y clínicos que requieren forta­lecer la evidencia, estandarizar protocolos y desarrollar medidas de resultado más adecuadas. Contar con guías de abordaje especializado, y en español, facilita el acceso de información actualizada a fisioterapeutas en América Latina.
