Abstract
INTRODUCION: usefulness of the measurement of adenosine deaminase in cerebrospinal fluid of children with tuberculous meningitis at the Hospital Universitario San Vicente de Paúl in Medellin, 2001-2003.
OBJECTIVE: to determine the diagnostic utility of measuring ADA in the CSF in children with suspected tuberculous meningitis.
MATERIALS AND METHODS: a retrospective study of diagnostic tests. Review the clinical records for demographic data, medical history, result of ADA and other test in CSF, of the children admitted at the Hospital Universitario San Vincente de Paul (HUSVP) between November 1, 2001 and May 31 of 2004 with suspected diagnosis of tuberculous meningitis (TBM). Patients were grouped according to the diseases most frequently encountered and compared the value of the ADA among these entities.
RESULTS: the mean CSF ADA activity was found to be significantly higher in CSF of TBM patients (18.4 UI/L), than in the CSF from non-TBM bacterial meningitis (14.6 UI/L), viral meningitis (13.3 UI/L) and from other neurological and no neurological disorders group with values under 7 UI/L. Using a cut off 7UI/L, the ADA test gave a sensitivity of 50 por ciento, a specificity of 78 por ciento, PPV of 35 por ciento and PNV 87 por ciento.
CONCLUSION: ADA is a tool to be used in conjunction with the diagnostic criteria of tuberculous meningitis to confirm or exclude the diagnosis. Because of it is an inexpensive and relatively simple test, justifies its application in clinical practice, taking into account that it is more specific than sensitive.
